Investigation Summary
On November 7, 2005, Employee #1 was greasing a food wrapping machine. This machine was custom made and no manufacturer information was available. The drive mechanism for the conveyor contained four sprockets with a 1-in. wide chain. This sprocket and chain mechanism was normally enclosed by a sheet metal cover. Each of the sprockets had a grease hole on the bearing pack around its axle. In order to grease these bearings and the chain, the cover was removed. Employee #1 used a grease gun to fill the grease holes in the bearings, and a brush to grease the chain while the conveyor was running. Then using a rag, he wiped the excess grease from around the bearings. Employee #1's right hand caught in the chain and sprocket nip-point and three fingers were amputated. Employee #1 was hospitalized.
