Investigation Summary
On June 23, 2005, Employee #1 and a coworker were installing curb, metal base frames, for the HVAC units. They were working on top of an approximately 20-ft high commercial building. Roof on this building had been completely sheeted and the framers had made 7 cutouts, HVAC penetration holes. These holes, measuring approximately 30-in. by 52-in. each, were protected with secured plywood covers. For each hole, process required them to remove a plywood cover, install the metal curb and then reinstall the plywood cover on top of the curb. The coworker, a foreman, was the lead in this process and the employee was helping him. After removing a plywood cover, the employee was putting it away, when he accidentally stepped into the open hole under the cover he had just removed. The employee fell about 18-ft 6-in. down onto concrete floor. The employee and the coworker were not wearing any personal fall protection equipment. The employee suffered multiple fractures to both of his wrists, pelvis and several ribs. The surgery was performed to repair the damage, and he was hospitalized for four days.
