Investigation Summary
On May 25, 2009, an employee and a coworker were mounting and inflating some truck tires in the service garage. The coworker was the lead mechanic and the employee was his helper. This accident involved a Michelin 14:00 R20XL truck tire using a multi-piece rim. This two-piece rim consisted of the main rim and a side ring. The main rim piece had a flange or lip for the tire on one side and 10 studs spaced equally, in a circular pattern, on the other side. The side ring had 10 holes to match the 10 studs on the main rim piece. After sliding the tire over the main rim piece, the side ring was placed through the 10 studs sticking out of the main rim and 10 nuts were installed onto the studs to make another flange for the tire. A rubber O-ring was used between the two metal pieces of the rim to make an air-tight seal. At the time of the accident, the coworker had just finished mounting and inflating this wheel assembly. The coworker asked the employee to check the pressure in the tire he had just inflated and put it away in the rack. The assembled wheel was lying on the floor in the horizontal position with its side ring and 10 studs or nuts facing up and it was inflated to about 80 psi of air pressure. At this time, the coworker had stepped a few feet away from the area, when he heard ratcheting sound of an impact wrench and soon after that the wheel assembly exploded. The side ring separated from the wheel and struck the employee in the chest and face area and then flew up making a hole in the ceiling and landed in the parking lot about 50 feet away from the launch point. The impact wrench was also thrown up, which made a separate hole in the ceiling and landed on the roof. The employee was also thrown about six feet away by the explosion. Two of the ten studs were dislodged from the main piece of the rim and all the nuts were scattered all over the shop floor. The employee suffered blunt force trauma to his head and chest and was pronounced dead within one hour.
