Investigation Summary

Investigation Nr: 201146180
Event: 09/23/2004
Employee Amputates Fingers While Manufacturing Windows

On September 23, 2004, Employee #1, while working alone, was pushing the end of a window sash or a surface planer. He used a push stick to push the sash over the cutter. Somehow, his hand came in contact with the cutter. There were no other observes. Employee #1 amputated his right middle and ring fingers at the first knuckle. His employer manufactures wooden windows and doors for residential construction. The surface planer was manufactured by Lartigiana Company (Equipment Number 70.) The planer is used for finishing the edges of window sashes. A wishbone-type swing guard is provided to cover the main cutter head. The edge of the window is suppose to be placed against a guide fence and slide over the rotating cutter. The swing guard automatically adjusts itself to cover that portion of the cutting head not protected by the material. A piece of extra lumber is used as a push stick to push the trailing end of the window sash over the cutter. He was hospitalized for over 24 hours.
