Employee is injured by glass panels

At 3:44 p.m. on July 20, 2004, Employee #1 was working for a general contractor performing major renovations to an elementary school. Coworkers had just removed various-sized panels and stacked them on a Perry PD-4 drywall cart and then moved on to work in a different area. Employee #1 had been working in another building and went looking for a drywall cart. When he saw the cart with the glass panels, he moved it forward without realizing the glass panels were not secured. The cart then fell over on top of him and he was struck by the falling glass. The coworkers heard the breaking glass but did not witness the accident. Employee #1 was hospitalized for lacerations and fractures.
