Employee Amputates Finger in Machinery

At approximately 5:24 p.m. on June 7, 2004, an employee was operating an Eagle Eaton Leonard, Model Number VB300, Serial Number 66-4116R, tube-forming machine. The employee was running the machine on automatic. On the floor, below the point of operation, were pressure sensor pads that acted as point of operation guards and shut the machine down when the operator stepped on them. The employee put a 14-in.long piece of tube into the clamp and dies area; the tube hung up. He was standing on the sensor pad, holding onto the piece of stock, when he accidently stepped on the foot pedal and activated the machine. The point of operation was not effectively guarded. The employee suffered crushing injuries to fingers on his left hand. Surgeons could not save the ring finger and did a complete amputation along with a partial amputation of the pinkie finger. He was hospitalized over 24 hours.
