Employee Amputates Finger in Machine

On June 6, 2004, an employee was being trained by a coworker in the process of setting-up a new die on a 250-Ton Alpha press, rebuilt by Gasbarre Products, under the Model 371-3015, Serial Number 2334.The employee was installing the fill ring, the lower part of the die, on a hydraulically adjustable bed on the press. In order to facilitate the installation, the employee asked the coworker to raise the bed a little. The coworker raised the bed by activating a switch on the control panel while the employee's right hand was still on top of the upwardly moving part. The lower part of this machine moved faster than intended and the employee's right middle finger was caught in between the moving part and the stationary part on the top. The employee amputated his right middle finger below the first knuckle. He lost about 1.5 inches of the finger. The employee was hospitalized and treated for the amputation.
