Employee killed after being struck by steel disks

On April 9, 2004, Employee #1, a production foreman, was working for a manufacturer of metal bearings used in naval vessels. A truck had brought a sea container of cargo from England to the company. The employee opened the right door of the container, then opened the left door. During transit, a pallet containing four steel disks weighing approximately 3,000 pounds had broken free and was leaning against the door. When the latch was freed, the weight of the steel disks pushed the door open, and the pallet fell out of the truck, landing on the face and upper torso of the employee. Employee #1 was killed.
