Employee's Foot Is Crushed and Amputated by Vehicle

At approximately 11:06 p.m. on February 16, 2004, an employee was working as an operator of a Capacity Trailer Jockey industrial truck Number 8402, used to reposition large trailers to be loaded or unloaded at the warehouse. Thinking he smelled smoke, the employee applied the parking brake and left his vehicle in gear while he got out of the cab. Then he raised the cab to view the engine area. When he lowered the cab, a hammer had slid under the accelerator, so when the cab was fully lowered, the engine was revved up to 2800 rpm. The right rear outside tire, dual tires on both sides, began spinning, catching the employee's foot underneath it and crushing it. The employee's foot was amputated at the point where employee's steel toe stopped protecting his foot. The employee was hospitalized and treated for foot amputation.
