Employee dies from fall

On February 16, 2004, Employee #1 and coworkers were applying stucco to the exterior of an 18-foot-high commercial building that was under construction. A two-tier tube and coupler scaffolding encircled the perimeter of the building, with the lowest tier measuring 14.17 feet above the ground. Additionally, there was a decorative granite strip, approximately 15 feet above the ground that required to be masked before stucco was applied. As Employee #1 sat on the lower tier of the scaffolding and was covering the granite strip with a plastic sheet and masking tape, he fell. For some reason, he had leaned back into the midrail of the guardrail and it came loose, causing him to fall. Employee #1 was transported to Western Medical Center in Santa Ana and pronounced dead. A detailed investigation revealed that there was no defect with the coupling or pin of the vertical member of the guardrail, but they were not properly locked. The scaffolding contractor set up the scaffolds three weeks before the accident and only a visual inspection was conducted prior to Employee #1 and his coworkers starting work.
