Investigation Summary
On November 5, 2003, Employee #1 was working in a bakery that manufactures cookies. Employee #1 was operating equipment used for sugarcoating the cookies. This equipment was specifically built for this operation and make and model of this equipment was not available. It consists of a metal tumbler, which is about 15 ft long and has a diameter approximately 4 ft. This cylindrical tumbler rolls on a metal frame with the aid of three sets of sprockets and chains. The nip-points created by these sprockets and chains were unguarded. The center of this horizontal tumbler is approximately 3 ft above the floor. Material is fed into this tumbler from a horizontal conveyor mechanism, which is approximately 4 ft long and approximately 3 ft high. This conveyor is driven by a set of chain and sprockets mounted on the side of the conveyor, which was not guarded at the time. Cookies are placed on this conveyor and they get sprayed with syrup, from a sprayer built on top of the conveyor, before they are automatically dumped into the tumbler. A hopper for adding powdered sugar is also built on top of this conveyor. A pipe and auger mechanism attached to this hopper dumps the powdered sugar directly into the tumbler on top of the syrup moistened cookies. While the cookies are tumbled around they are also being pushed to the other end of the tumbler by the fins built inside the tumbler. Cookies and the excess sugar are then collected from the other end of the tumbler manually into separate containers. Excess sugar is then added back into the front end of the tumbler for re-use. Re-cycled sugar or fresh sugar can also be added directly into the front end of the tumbler bypassing the hopper. At the time of the accident, Employee #1 was dumping fresh sugar directly into the tumbler from a bag, when his left hand was caught in the unguarded chain and sprocket for the conveyor in front of the tumbler. Employee #1 severed his left little finger at the bottom of the nail and lost approximately 0.5 in. of this fingertip. Employee #1's left ring fingertip lost a little flesh only. Employee #1 and the severed pieces of his fingers were taken to the hospital, but they were not suitable for re-attachment. Employee #1 was hospitalized for a week.
