Investigation Summary
At approximately 11:00 a.m. on October 3, 2003, Employee #1 was replacing a fluorescent light fixture in a warehouse. A coworker attached a GAI basket with a rated capacity of 1,000 pounds, a 45-inch top rail, a mid rail, and a locking gate to a 2475-pound Nissan propane powered forklift, Model Number CPH01A15PV. The hanging light fixture was 17.5 feet above the floor level. Employee #1 was in the basket and the coworker raised him up approximately 9 feet. The coworker had to use the bathroom and locked and shut down the forklift, leaving Employee #1 suspended in the basket. Employee #1 became impatient and stood on the mid rail to gain a greater working height. He lost his balance and fell out of the basket, landing on his right shoulder. He sustained multiple fractures to his right shoulder and upper arm and was hospitalized. He was not secured to the boom by a safety belt and lanyard.
