Employee hand crushed in a machine

At approximately 8:05 a.m. on April 19, 2003, Employee #1 of Hot Shot Plastic Inc. was working alone, trouble shooting a JACO III, Model Number 259M, Serial Number 1185, injection molding machine. The machine was experiencing hydraulic problems during the recovery phase. The machine continued to re-inject or extrude, causing a tail to form on the bottom of the pot. The machine had a sliding gate guard with limit switches and the guard extended over the dies. When the pot is ejected, it drops downward and passes a sensor. The sensor causes the machine to recycle. Employee #1 removed the guard and reached in with his left hand to pull off a pot. The pot fell down; the sensor told the machine to recycle. Employee #1's hand was caught between the dies. He sustained a crushing injury to his left hand. He called for emergency medical services and was transported to the UCI Medical Center. Employee #1 was hospitalized 5 days for orthopedic reconstruction of the hand.
