Investigation Summary
On January 10, 2003, Employee #1 was using a broaching machine, Model VT-315B, Serial Number 1673, manufactured by Pioneer Broach Company, to create serrations on the outside of small parts, which had outer diameters of 0.375 inches and inner diameters of 0.25 inches. He encountered a problem with the machine and that required adjustments to the travel limits of the broach ram that moved the rod, with the mounted parts, up and down inside the machine. Employee #1 asked a coworker, a maintenance/setup man, to fix the problem. The coworker made some adjustments and told Employee #1 that the machine was fixed. After the coworker left the area, Employee #1 noticed the same problem occurring with the machine. This time, Employee #1 decided to make the adjustments himself. He opened up a small door on the machine to access the adjustment knobs while the machine was still energized. He had his left hand in the machine when the ram activated. All the fingers on Employee #1's left hand were crushed. He was transported to Western Medical Center for treatment.
