Investigation Summary
At approximately 10:30 a.m. on December 2, 2002, Employee #1, a union ironworker, was installing single span decking on beams 14 ft above the floor. As Employee #1 started to tack the decking down, he realized that the sheet was cantilevered in the wrong direction. Employee #1 stood up to re-check the plans, and as he shifted his weight, he stepped on the unsupported end of the decking and fell. Employee #1 suffered a dislocated right elbow and fractured right wrist. Employee #1 was hospitalized.
