Investigation Summary
At approximately 8:30 a.m. on August 28, 2002, Employee #1 was using a Clark electric forklift (Model Number NP30030, Serial Number NP246-0062-5601) to place a pallet on a 20-ft-high warehouse shelf when the pallet slipped. He straddled the forks and then had a coworker raise him so he could reposition the pallet. In the process, the coworker retracted the forks, causing Employee #1 to lose his balance. He grabbed onto the retracting mast and his left hand became caught between the mast and the forklift cage. Employee #1 was hospitalized for surgical reconstruction of the hand; however, his middle finger had to be amputated at the first joint.
