Investigation Summary

Investigation Nr: 201143161
Event: 11/09/2002
Employee's toes amputated after being crushed by trailer

On November 9, 2002, Employee #1 was working for Jimco, a business that provided railroad support services, including assistance to railroad companies during derailments, and the moving of stalled locomotive equipment. A Union Pacific locomotive engine had stalled on the railroad tracks. Employee #1 and a coworker were bringing a tractor to pull this locomotive. They hauled this tractor to the site on a low bed trailer. They had unchained the load to roll the equipment off the trailer. The coworker lowered the trailer using a control lever provided for this purpose. Employee #1, who was standing on the other side of the trailer, was not aware that the trailer was being lowered. The trailer came down on Employee #1's right foot and he suffered a crushing injury. Employee #1 was admitted to UCI Medical Center in Orange and surgery was performed to save his toes. But it was not successful. Employee #1 was hospitalized for about a week at UCI Medical Center and then was transferred to Loma Linda University Hospital, where all four toes except the big toe were amputated. Employee #1 was hospitalized at Loma Linda University Hospital for and a week and half.
