Employee amputates fingers on pipe swedger

At approximately 1:45 p.m. on September 10, 2002, Employee #1, a production laborer at a motorcycle exhaust manufacturing facility, was assigned to swedge the end of steel exhaust pipes on a Vickers Swedger, Model Number CJ 040178. He had his left hand on the end of the pipe when he activated the machine. As the pipe was driven against the steel stop, all four of his fingers were amputated at the palm.
