Electrocution

On July 31, 2002, at approximately 9:00 a.m., employee #1 was in the process of replacing a existing ballast in a 4 ft. long florescent light fixture. A portable light had been provided for illumination. Employees chose to leave lights on because t hey could see better. While placing a wire not on the hot wire in the fixture being worked on, the supervisor turned the lights back on which resulted in employee #1 receiving an electrical shock. Employee #1 died at approximately 9:27 p.m. on July 31, 2002 as a result of the electrical shock.
