Employee Is Killed When Pinned Between Truck and Trailer

At approximately 9:00 a.m. on October 20, 2003, Employee #1, an equipment operator, was positioned between a flatbed trailer that housed a combustion engine driven bailer, and a Hyster fork truck, to hook up battery cables for the bailer. The engine for the bailer was positioned on the front end of the flatbed trailer and was approximately 8 feet above the ground. Employee #1 was standing on a plastic milk crate or on the forks of the fork truck in order to reach the battery cables. The fork truck moved forward and pinned him between the truck and trailer, killing him. There were no witnesses to the event.
