Employee kille when struck by steel beam

On May 12, 2003 an employee working for a steel erection company was attempting to weld one end of a beam by a 16ft steel I beam weighing approximately 768 lbs. the opposite end of the beam broke loose from its weld falling from the forklift it was supporting striking and killing the employee.
