Employee killed in fall when roof joists tilt

Employee #1 was at the roof level of a building under construction, unhooking roof decking bundles as they were landed by a crane. The joists on which Employee #1 was standing became unbalanced by a roof decking bundle and tilted, causing him to fall head first approximately 36 feet to a concrete surface. He sustained fatal head injuries. Employee #1 was wearing a safety work belt with lanyard but the employer had not supplied a safety net or safety line for fall protection.
