Investigation Summary
On March 8, 2002, Employee #1, a helper, and an electrician/foreman arrived at a residential construction site to complete the installation of a pump start relay. The work required a new 220-volt line to be pulled from the existing electrical panel over to the junction box that contained the pump start relay. The foreman was doing some work at the truck when Employee #1 removed the cover from the junction box and started to insert a fish tape through the conduit in preparation for pulling the new wires. The fish tape contacted an exposed and energized 120-volt wire from a timer that was also located inside the junction box. Employee #1 received an electric shock and fell to the ground, unresponsive. Coworkers at the job site administered CPR until the Fire Department arrived. Employee #1 was transported to the hospital, where he was pronounced dead. The exposed wires located in the junction box were not tested to determine if they were live, and the electrical circuit for the timer was not turned off, and then locked out or tagged out, at the disconnect switch at the electrical panel.
