Employee Died as a Result of a Fall

On February 14, 2002, Employee #1 had taken a shorter route to his forklift truck, when he tripped over a 13-in. beam that was mounted to the floor and hit his head on the forklift truck. Employee #1 suffered head injuries and bruises, contusions and abrasions and was hospitalized in a rehabilitation center. One week after being release from the rehabilitation center Employee #1 passed away on May 15, 2002, at his home.
