Employee killed when crushed by press

Employee #1 was operating a Standard Air Clutch Platen Die Cutter, Model II, Serial Number P 93195. The machine was set in auto mode and the employee was placing a 28-inch by 20-inch by .80-inch thick part on the inclined lower plate that was 43 inches long across the front and 33 inches deep/wide. The part was positioned against two locator pins near the bottom of plate and a third locator pin on the left of the plate. As the machine cycled, the plate rose up to the die holder which cut off a portion of the part. It is believed that the cut-off part slipped into the opening at the back of the machine between the plate and the upper die holder. The employee reached across the plate and into the opening between the plate and the die holder with her right hand to retrieve the part. As the machine cycled, it caught her hand in that pinch point. The employee initiated the emergency stop bar with her knee and stopped the machine. A coworker came to help and told the employee that he was going to turn off the power at the back of the machine before he tried to get her out. As he went to the back of the machine, he heard the machine cycle which crushed and killed Employee #1. The coworker turned to see another employee at the front of the machine. The other employee had hit the start button which reactivated the machine.
