Investigation Summary
At approximately 5:43 p.m. on April 15, 2002, Employee #1 was jacking up on one side of a mobile home when the jacking shifted and fell on top of him. Employee #1 tried to back out from under the home, but a fence post prevented him from escaping. Employee #1 was asphyxiated when the mobile home fell on top of him.
