One killed, one injured by falling lamp and cage assembly

At approximately 8:00 a.m. on or about March 3, 2002, Employees #1 and #2 were using a postable winch to lower an aviation lamp that had become unsynchronized from the top of a 195 ft tower. Employee #1 connected the lowering wire to the winch, but apparently did not made a positive and secure connection. He was starting to lower the lamp when the connection came apart, and the lamp fell toward the two employees, who were on a platform at the 130 ft level. Employee #2 attempted to push the falling lamp and supporting cage away from Employee #1, but was struck by part of it, fracturing his left arm. Employee #1 was directly below the approximately 65 lb assembly. He was struck on the head and was killed.
