Investigation Summary
At approximately 4:50 a.m. on December 24, 2002, Employee #1 was troubleshooting a faulty elevator gate switch. He was wearing safety glasses, a hardhat and safety shoes. However, the elevator did not have interlocked access panels. As he repaired the switch, the elevator turned on and started to travel down the shaft. The elevator struck him in his head, causing traumatic head and neck injuries, and killing him.
