Investigation Summary
On October 17, 2002, Employee #1 was placing pins into a machine's point of operation, when her thumb became pinched by a side clamp. After calling for help, a coworker hit the activation button, causing the ram to descend, which crushed two of Employee #1's fingers. She was hospitalized for the amputation of both fingers.
