Investigation Summary
On January 15, 2002, Employee #1 and another employee of the Boeing Company were working on a 3000-ton aircraft structure aluminum forming machine (TC-3000C) stretch press, to rebuild a leaking lift cylinder. (The work had been begun and continued on previous shifts.) The two employees were removing the collar, which weighed approximately 100 pounds from around the hydraulic cylinder. During this process, the stored hydraulic energy explosively released and propelled the collar which struck and killed Employee #1.
