Eight employees injured while investigating a furnace outage

On October 28, 2002, eight employees were attempting to remove an access door from a furnace to investigate a blast furnace outage. For some reason, an explosion occurred when the door was opened. The Blast Furnace Operator is responsible for ensuring conditions are safe before allowing access to the space. Further, flares are to be continuously thrown into the furnace to minimize excess gas from building up. During the preparation time of 30 to 45 minutes, flares were not thrown and the operator had no procedure in place or any way of communicating with the personnel prior to opening the access door. All eight employees received injuries to include bruising, burns, abrasions, and contusions, but only one was hospitalized due to an eye injury.
