Employee killed when struck by bale of paper

At 3:27 p.m. on March 4, 2002, Employee #1, a sweeper employee in a paper recycling facility, was struck by a bale of paper weighing approximately 1,300 pounds. Employee #1 was working his normal work shift from 7:00 a.m. to 4:30 p.m. Employee #1, an immigrant from Mexico, spoke Spanish and had worked at the facility from September 4, 1998 until October 8, 1999, when he left the US to return to Mexico. Employee #1 returned to work at the facility on December 12, 2001. The bales of paper were in storage. Employee #1 was sweeping scrap paper off of the floor directly in front of a stack of four paper bales, approximately nine feet high, when the bales fell down and struck Employee #1. Employee #1 suffered blunt impact wounds to the head, torso, and extremities. No other employees witnessed the incident. Employee #1 was found by a coworker, and Emergency Medical Services was called. Employee #1 was pronounced dead at approximately 4:10 p.m. on May 4, 2002.
