Investigation Summary
At approximately 8:18 a.m. on February 28, 2004, Employee #1, while in the process of cleaning exhaust fans on the roof of Elkton Die Casting LLC, fell 35.5 ft through an opening in the roof created by the removal of the exhaust fans and landed on a machine inside the factory. The employee was killed.
