Employee killed when struck on the head by falling beam

Employee #1 was moving a 20-ft steel beam stored on five racks. He was backing up on a forklift and misjudged the proximity to a steel rack behind him. He struck a beam with the upper portion of the forklift, causing the beam to fall on his head, killing him.
