Employee falls from unguarded scissor lift and dies

Employee #1 and a coworker were working from a scissor lift approximately 9 ft high with no fall protection on one end of the platform. They were using a nail gun to install a nailer strip onto a canopy structure. Employee #1 was holding the nailer strip in place and fell from the platform and died.
