Investigation Summary
A driver parked in front of the maintenance shop, got out, and spoke with the shop foreman about a possible fuel leak on a truck. The fuel leak had been addressed on this same truck two days prior. Employee #1, a mechanic, crawled under the truck while it was running. The shop foreman directed the driver to pull the truck around to the rear of the maintenance shop. The driver returned to the truck, got in, and backed up, running over Employee #1. Employee #1 was killed.
