Investigation Summary

Investigation Nr: 201040888
Event: 01/17/2002
Employee's Leg Crushed When Run Over By A Fork Lift

At approximately 2:10 p.m. on January 17, 2002, Employee #1 was pushing a wheelbarrow full of debris, was approaching the trash pile from behind the fork truck. The driver did not notice Employee #1 approaching from behind. After dumping his trash, the forklift operator put the forklift in reverse and backed up approximately 5 feet to allow room for the forklift to clear the debris pile. At this time, the driver engaged in a conversation with another coworker who was to the left of the driver. During this time, Employee #1 had reached the debris pile and started to dump the wheelbarrow full of debris onto the trash pile. The driver had not noticed Employee #1 come from behind and start working next to the vehicle. After finishing a brief conversation with a coworker, the driver made a sharp left hand turn and pulled forward approximately 5 feet. The driver then heard a scream and immediately stopped the forklift. The driver looked down to his right hand side and saw Employee #1 on the ground, apparently with his left leg under the right rear wheel of the forklift screaming in pain. The driver exited the vehicle on the left side and started yelling for assistance. At that time, a coworker came from his truck and ran around the right side of the forklift. There, he saw Employee #1 with his left leg pressed tight up against the back side of the right rear wheel. The leg, with a compound fracture, was bleeding and Employee #1 was screaming to get the forklift off his leg. A coworker drove the truck forward approximately 5 feet and then shut the truck off. By this time numerous other employees had arrived on the scene and started rendering first aid, including the application of a tourniquet made from an employee's belt. Paramedics, fire department and police arrived on scene. Employee #1 was transported to UMC medical center trauma to repair his leg. Employee #1 suffered abrasions and a compound fracture on his left leg, requiring multiple surgeries.
