Investigation Summary

Investigation Nr: 201572351
Event: 11/15/2002
Employee Killed When Struck by Crane Boom

At approximately 12:52 p.m. on November 15, 2002, workers were repairing a drilling rig, a Watson 3100 CM, on the ramp to an excavation. The employer did not ensure workers were protected, exposing them to various hazards, including hazards of employees being crushed by, or caught between, the drill rig boom, work trucks parked on the ramp, and the narrow width of the ramp. While working on the kelly-bar hoist cable and giving hand signals to move the boom to the back of the work truck, Employee #1 was struck by the boom of the crane when it swung the wrong way. He was crushed between the unsupported wall, approximately 7.4 ft high, and the boom of the drill rig. Employee #1 was killed.
