Employee Dies Choking On A Sandwich

At approximately 7:00 a.m. on September 27, 2001, Employee #1 and a coworker were preparing to start work. His coworker went to his truck while Employee #1 began working while eating his sandwich. His coworker heard the breaking of a 2-by-4 wood that was supporting the cover over a shallow trench. Employee #1 was choking on his sandwich and then fell into the excavation. Employee #1 died.
