Investigation Summary
At approximately 6:30 a.m. on February 6, 2002, Employee #1 of the U.S Department of Defense was unloading boxes from a wooden pallet to warehouse storage racks. He was not wearing the safety belt and lanyard that was attached to the material handling truck. Several planks on the wooden pallet were broken and the pallet possibly gave way causing Employee #1 to lose his balance and fell approximately 11 ft 2 in. from a wooden pallet elevated on the forks of a high lift material handling truck and was killed.
