Employee injured in fall from pallet

At 11:45 a.m. on October 18, 2001, Employee #1 was in the process of looking for a box of computer floor pedestals. He saw a box on a shelf, approximately 9 feet from the floor, on a storage rack. In front of the rack was a palletized load of computer floor tiles, approximately 5 feet in height. Employee #1 jumped up on the pallet and reached into the box. Unfortunately he stepped back and slipped from the pallet. Employee #1 landed on both heels, fracturing both heels. Employee #1 was hospitalized with fractures.
