Investigation Summary
On December 18, 2002, Employee #1, a truck driver, arrived at a worksite to deliver an empty refuse container and removed a full one. With the full container loaded on the flat bed truck, materials were spilling out the opening of the container onto the ground at the rear of the truck. Several attempts were made to clean-up and stop the spillage. Employee #1 was found dead, because he was pinned between the rear of the container and the ground.
