Investigation Summary
At approximately 2:20 p.m. on October 4, 2001, Employee #1, a utility box handler, was standing near the doorway with his back facing the Raw Stock Storage Area. The doorway connected the Raw Stock Storage Area to the Raw Stock Receiving Area. While the forklift was backing out of the Raw Stock Storage Area, Employee #1 turned around towards his right until he saw the forklift. The left rear tire of the forklift ran over Employee #1's right foot. Employee #1 broke his right foot, broke his lower right leg, and broke his left wrist. The forklift operator failed to look in the direction of travel, and his employer did not have a policy which restricts pedestrian traffic through the Raw Stock Receiving Area, while trucks were being unloaded. The Raw Stock Receiving Area was somewhat cone-shaped and was approximately 25 feet long and was 8 feet 1 inch wide at its West end and 21 feet 8 inches wide at its East end.
