Investigation Summary
At approximately 3:30 p.m. on November 21, 2001, Employee #1, an employee of Oregon State Parks and Recreation Department, was transporting concrete sign posts using a three-wheel loader. Employee #1 drove the loader to the right shoulder. The loader began to tip and eventually rolled over, crushing and killing Employee #1.
