Employee dies from fall off fourth floor balcony

At approximately 8:45 a.m. on February 16, 2002, Employee #1 was installing hurricane clips to the roof of a balcony. The balcony was located on the fourth floor of a new apartment building under construction and did not have guardrails. During the installation, Employee #1 was not wearing fall protection. To attach the hurricane clip between the truss and the truss support of the roof, he stood on a bucket. While nailing the hurricane clip to the roof, the bucket tipped over under his weight. He lost his balance, but grabbed the truss support to stabilize himself. However, he could not maintain his hold on the truss support and fell from the balcony to the ground. The impact upon the ground caused blunt force trauma to his abdomen and extremities. He died due to hepatic and renal complications resulting from the severe injuries suffered in the accident.
