Investigation Summary
On September 17, 2001, Employee #1 was working on the roof of a two-story single-family home. Employee #1 climbed a 16-foot ladder that was set up at the garage area, with a ground to eave height of 12 feet. While getting off the ladder to the roof the ladder shifted. Employee #1 fell 12 feet to the ground, fracturing his back and pelvis, and was hospitalized.
