Investigation Summary

Investigation Nr: 202328639
Event: 09/13/2001
Employee's Foot Struck By A Fork Lift

On September 13, 2001, another employee backed up and struck Employee #1 with a forklift when Employee #1's ankle became caught between the backing forklift and a wooden pallet. Employee #1 was standing next to the pallet picking and stacking boxes of the product on the pallet. The forklift operator revealed that he had just picked up a pallet of product in a side aisle and had turned backing down the main aisle in the direction of Employee #1, who was also working in the same main aisle. He looked over his right shoulder and started to proceed down the aisle to maneuver around Employee #1's pallet. He then glanced over his left shoulder and heard Employee #1 scream in pain. He then pulled the forklift forward and went to assist the Employee #1. Employee #1 was transported to a local hospital to treat his fractured ankle.
