Investigation Summary
On August 23, 2002, Employee #1 was standing on a stepladder to install recessed lighting that was positioned next to a floor opening, which was 23.25 feet above a concrete basement floor. For some reason, Employee #1 fell off the ladder and through the hole in the floor, striking his head on the basement floor. He suffered a concussion and was killed.
