Employee killed after falling 50 feet

On June 7, 2002, an employee of Brasfield and Gorrie Llc was trying to unhook a nylon strap which had gotten caught on the edge of a skip-pan. The pan had just been flown up by a tower crane and rested on a concrete beam. The beam was approximately 3 feet wide. The employee had just used a 4-in. by 6-in. by 4.5-in. board to level the skip-pan on the beam. He was to put scrap wood and debris from the construction of the stairwell in the skip-pan. The employee lost his balance; he and the skip-pan fell approximately 50 feet to a lower level. He was killed by the fall.
