Employee Is Electrocuted and Is Killed

On May 7, 2002, Employee #1 was installing a water fountain in a pond. The fountain did not work after the power was turned on. The power was turned off and the employee volunteered to swim back out to the fountain to listen and see if the motor would run after the power was turned on. The employee gave the signal for the power to be turned back on and was last seen going under. Employee # 1 was killed. The cause of death was electrocution due to the absence of a ground fault circuit interrupter at the fountain.
